As a Supported Living Facilitator, youll be encouraged to grow, develop and feel empowered to make the everyday easier for the people we care so passionately about. Lets make a meaningful difference, together! Responsibilities Youll provide care to clients living in the community, supporting their independence and improving their quality of life. Spend time on the road completing scheduled visits and interacting one-on-one with clients and their loved ones within their homes. Complete activities including: Availability Tuesdays (weekly) and Fridays (fortnightly) 9am-5pm In-home personal care, daily living assistance including showering, personal hygiene, meal preparation, house cleaning, laundry & ironing Must be available to drive between clients with your own vehicle Providing care in accordance with individualised Care Plans Supporting community-based recreational and social activities Transportation and assistance with shopping and appointments Qualifications and Requirements Able to work effectively with others in a team-oriented environment and provide excellent support to clients and co-workers Able to effectively communicate in spoken and written English Experience in the Disability, Aged and Community sector is ideal but not essential; positive attitude and willingness to learn A current NDIS Worker screening check Current CPR and First Aid Certificate (required before you start) Open Drivers License and access to vehicle Car Insurance in your name What makes a difference for you Flexible work arrangements to support work-life balance NFP Salary Packaging: Save tax and increase your take home pay by salary packaging everyday living expenses and bills (up to $15,900) and meal entertainment (up to $2,650) per annum A recognition and rewards platform with employee discounts at retailers, and programs for wellbeing and assistance 12 weeks paid parental leave and 2 weeks paid partner leave after 6 months of service Diversity and inclusive leave, including paid gender affirmation leave and cultural leave options Career development opportunities to challenge yourself and grow A positive and inclusive team based on respect and shared values Wed love you to be part of our UnitingCare family Joining our FaDS team, youll be welcomed as part of the UnitingCare family.
