This position provides direct clinical care while also acting as a clinical leader within the service. You will manage a clinical caseload delivering generalist nursing services including wound care, continence, medication management, diabetes support, and palliative care. In addition, you will play a key role in mentoring less experienced nursing staff and contributing to the ongoing development of clinical practices, procedures, and quality improvements within the team.Key ResponsibilitiesConduct comprehensive nursing assessments using evidencebased frameworksDevelop and review clientcentred clinical goals and care plansDeliver nursing interventions including wound care, medication management, pressure care, continence care, and dementia screeningEducate clients, families, carers, and support workers on care plans and interventionsWork collaboratively within a multidisciplinary team to support holistic careMaintain accurate clinical documentation, reporting, and billing recordsContribute to service planning, quality improvement, and risk managementProvide guidance, mentoring, and professional support to nursing colleaguesSupport the development of clinical practices, procedures, and protocolsRequirementsThe successful candidate will require National Police clearance prior to appointment (completed by 3Bridges), valid NSW Working With Children Check clearance and current NSW driver''s licence.About 3Bridges3Bridges is a dedicated For Impact organisation that aims to enhance the wellbeing of individuals across all life stages.
