The successful person in this role will have experience providing homebased rehabilitation to patients alongside experience in team leadership, stakeholder management and supervision.This role includes but is not limited toProvision of occupational therapy assessment and intervention following an illness or injury in the patients'' home / Jewish Care Windsor.Liaison with service providers and completion of NDIS applications as required.Team Leadership and Coordination.Close multidisciplinary team collaboration and communication including attendance at journey boards and long stay meetings.
