You''ll support patients across a range of programs, including HCP & CHSP, PHI, and privately funded patients.What You''ll DoProvide clientcentred services such as home assessments, equipment prescription, and home modificationsSupport clients'' posthospital discharge to enhance safety and reduce the risk of readmissionDevelop and run group and individual programs for residential aged care clientsPrepare clear, practical reports that directly support care deliverywithout the burden of traditional bureaucratic documentationBe AHPRA registered (Full or Limited Remote Level)New graduates are welcome; experience in community or aged care is a plusPromote health and wellbeing through clientcentred careWork independently and travel across Hobart (vehicle required; travel reimbursement provided)Not based in Hobart?
