Based at our Home Care Office in Chinchilla or another local office, youll manage a caseload of Support at Home and Commonwealth Home Support Program (CHSP) clients, coordinating care services and using your knowledge to make a real difference in our clients lives. Key responsibilities Develop personalised care plans in accordance with assessed needs and clients wishes (and families where appropriate). Develop budgets with clients, utilising funding to meet their needs effectively. Coordinate delivery of services such as personal care, meal preparation, cleaning, and transportation. Monitor implementation of care plans and adjust as needed. Monitor client funding, take corrective action to ensure services are provided within funding; manage any unspent funds. Purchase and manage clients consumables, ensuring goods are supplied as required. Ensure CHSP services are provided in accordance with CHSP guidelines. Promote clients rights, preferences and independent decision making. Model professional behaviours and the Known & Loved framework; support our Support Workers. Conduct regular reviews and reassessments to ensure needs are met effectively. Provide information and support to clients and families regarding available services and resources. Liaise with healthcare professionals, service providers, and community organisations to ensure coordinated care. Maintain accurate records of care management services and document client interactions and service provision. Participate in out-of-hours on-call on a rostered basis. What will you bring to the role? Experience in a similar role within Aged Care or Disability Services Qualifications in Nursing or Allied Health (desired, not essential) Ability to adapt to changing situations and needs Practical and solution-focused approach to problems Strong organisation and time management; ability to prioritise Attention to detail, ensuring accuracy Ability to follow processes and create efficient work practices Reliability: consistent in meeting deadlines and commitments Excellent communication skills with clients and team members Exceptional interpersonal skills and the ability to relate to a variety of people Who is Southern Cross Care Queensland?
