The OpportunityWe are seeking an experienced and motivated Nurse Practitioner to join our client''s innovative and committed Residential In Reach Aged Care Services team.This is a part-time to full-time role offered on a fixed-term contract for 24 months.The CompanyOur client is a leading rural and regional health provider in Victoria with a vision: to create Australias healthiest rural community.They provide high quality, safe and person centred healthcare, working together with the community, consumers and carers to continually enhance their services.Backed by dedicated staff, passionate volunteers and strong community connections, this innovative organisation offers a fulfilling environment where every team member is valued, supported and empowered to make a real difference to the health and wellbeing of rural communities.The RoleAs an In Reach Nurse Practitioner, you will:Provide advanced clinical care, including assessment, diagnosis, treatment and management of residents acute and chronic conditions in consultation with the GP and the multidisciplinary team.Prescribe medications and order/interpret diagnostic tests in line with professional scope and legislative standards.Develop and implement individualised care plans in collaboration with residents, families, and the multidisciplinary team.Lead, mentor and support nursing staff to promote best practice aged care.Ensure compliance with clinical governance, quality standards and safety protocols.Advocate for residents wellbeing, independence and quality of life while promoting health education and preventative care.Collect and analyse RIR data and KPIsThe Best BitsExcellent salary packaging options, including meals and entertainment and novated leasingBe part of a high performing teamFree car parkingAccess to internal and external professional training and development opportunitiesSupportive culture that promotes staff health and wellbeingAbout YouTo be successful in this position, our ideal candidate:Hold current Nurse Practitioner registration with AHPRA and relevant endorsements.Have extensive experience in aged care, including managing complex and chronic health conditions.Relevant post graduate qualifications in aged care, health care or a related field.Is proficient in all Microsoft Office applications.Commitment to the WDHS Values of Integrity, Innovation, Collaboration, Accountability, Respect and Empathy and ability to exhibit behaviour which reflects our values.Ability to work collaboratively and cohesively with colleagues, supervisors, and otherstakeholdersCurrent Driving LicenceWillingness to travelPlease note:Successful applicants will need to hold, or be able to obtain, a current National Criminal Police Check.
