AnglicareCQ is seeking a compassionate and proactiveSupport Facilitator - Outreach to join our team and deliveressential support through The Way Back SupportService.In thisrole, you will provide assertive, timely psychosocial support toindividuals who have experienced a suicide atempt or suicidalcrisis. With a strong outreach focus, you''ll work across a hybridmodel of telehealth, phone-based support and regular face-to-faceoutreach - inclduing monthly visits to Far West communities. Theposition centres on proactive and persistent engagement, reducingbarriers and ensuring access for people living in remote and veryremote locations.ResponsibilitiesEngageparticipants facing isolation or access barriers withpersistent, flexible outreach.Completeassessments, safety plans, support plans, and facilitatewarm referrals.Maintainaccurate records, improve service processes, and stayinformed on service systems and pathways.Mandatory RequirementsCertificate IVqualifictions in a mental health or relatedfield.Who we areOurtruly local team of professionals have worked with people inCentral Queensland for over 40 years to support them in livingtheir best life.
