Comprehensive mental health, community health and dental services are delivered through various programs across the Barwon South West Region, including Torquay and Colac.About The RoleThe Respiratory Liaison Nurse plays a key role in supporting the delivery of high-quality, patient-centered care for individuals with respiratory conditions. This position involves assisting with the management, education, and discharge planning of respiratory patients, ensuring they receive comprehensive care across both hospital and community settings.In this role, you will provide clinical leadership within Barwon Health, ensuring the smooth and effective coordination of services. You will use a problem-solving approach to manage patient access, focusing on maintaining high standards of care and achieving positive patient outcomes. You will also be responsible for identifying policies, procedures, and guidelines that require updates based on the latest evidence-based practices.As a Respiratory Liaison Nurse, you will offer expert advice and education to both hospital and community-based respiratory patients. This includes collaborating with external stakeholders to ensure coordinated care for patients, and making referrals for individuals in the community to ensure proper support upon discharge. You will also coordinate care for patients using home oxygen, CPAP therapy, and managing conditions like asthma and COPD.
