About This OpportunityAs a Care Partner, you will be responsible for overseeing and coordinating all aspects of client care, ensuring a seamless and positive experience for both potential and existing clients. This position provides leadership and guidance in managing client interactions, working closely with the office team to meet the needs and best interests of the client while achieving key operational goals.Why join us? A rewarding career where you can make a positive difference in the lives of seniors and their families.Key ResponsibilitiesPlanning, managing, coordinating, and reviewing client goals.Actively communicate with team members and clients to develop care plans.Formalise agreements with the client and client''s family about the care services to be provided and costs involved.Accurately document all interactions held with client, family or other stakeholders about the client and share relevant information with appropriate team members including feedback about caregiver performance.Meeting with clients and their families face to face to discuss and review current care plans and goals.About YouQualifications in Community Services preferred.Aged Care and Case Management experience a must. Experience with Home Care Packages, Support at Home and Aged Care Case Management advantageous.Great customer service manner. Strong administration skills and computer proficiency required.
