Calvary St. Pauls Aged CareClinical Care Coordinator Permanent Full TimeLocation: Cundletown, NSW, Australia 2430Employment type: PermanentClosing: Jan 23 2026Start: Mid January 2026 but available to start sooner for the right candidate.Hourly rate: $53.31 per hour (EA).About The RoleThe Clinical Care Coordinator role is responsible for leading both the Clinical and Care teams across the home, in consultation with the Home Manager, to ensure the needs of each resident are being met and the team understands who the residents areincluding their identity, culture, diversity, beliefs and life experiences.You will play a key role in the clinical governance of the home and may be required to undertake a specialist clinical portfolio to support the home''s needs, examples including education, palliative care and infection control.You will also work in accordance with the mission and vision of Calvary and actively participate in developing a culture that promotes Calvary''s values of healing, hospitality, stewardship and respect.Key ResponsibilitiesEnsure compliance with legislative and statutory requirements to enable us to continue to deliver a high standard of care, services lifestyle for our residents.Coordinate the delivery of quality clinical services in line with a residentcentred model of care and the Australian Aged Care Quality Standard.Manage and provide efficient, effective and compassionate care planning and assessment.Ensure effective care planning processes and mechanisms to measure resident outcomes.Coordinate communication between the care/clinical staff, RN and relevant health professionals to ensure continuum of care to the residents.What You BringRegistered Nurse with current Registration with the Australian Health Practitioner Regulation Agency (AHPRA).A minimum of 3 years'' experience in a management role within the aged care sector with a successful and proven track record.High level of commitment, integrity, honesty and trustworthiness.Demonstrated knowledge and understanding of the aged care sector with the ability to identify changing trends and to ensure best practice.An effective listener with the ability to communicate at all levels both orally and in writing with excellent presentation skills.Understanding of and ability to manage human resource issues with effective negotiation skills.Ability to understand and use information technology and programs such as Word, Excel, etc. and to learn new skills as required.Working knowledge of WH&S and Infection Control.BenefitsNFP salary packaging benefits, discounted health insurance and gym memberships.Paid parental leave.Professional membership to APNA, paid for by Calvary.Training, development pathways and career opportunities.Flexible hours that make sense for you.Additional InformationAs part of the application process, you will be required to complete preemployment checks which may include reference checks, work rights, criminal history check and a health evaluation to ensure suitability for the role.You may be required to provide evidence of immunisation as required for your role including influenza, tuberculosis screening, hepatitis, diphtheria, tetanus and varicella.At Calvary, our staff matter.
