We provide navigational services to National Disability Insurance Scheme (NDIS) and My Aged Care Home Care Package recipients. By staying focused on navigation and administrative services, we support our clients to effectively orientate complex and fragmented service systems. We act as your primary service provider by helping our clients understand the system and exercise informed decision-making to enable choice and control. We provide navigation and administrative services in the following areas:Role overviewThis Service Coordinator role is an entry level role suited for a recent graduate or someone looking to do a career change. The role is aimed at enhancing the lives of individuals living with disabilities, mental ill-health and/or aging. The primary responsibility of this role is to develop, implement, and monitor individualised support plans for participants with complex needs, ensuring these plans align with the legislative frameworks, to effectively meet participant goals and safeguard them.Key approachThrough their sound knowledge and ability to solve complex service delivery problems and identify opportunities in a complex support environment, the Senior Service Coordinator can navigate multiple service systems, manage clients with interrelated needs and make suitable recommendations to our clients to better meet their individual needs. You will support clients and their carers to manage their support plans, and community and mainstream services, with a proactive, outcome-led approach to help them build capacity, chart a path to independence, acquire new skills, receive quality care and/or keep safe. You will have the ability to:Build rapport with all stakeholders quickly while having the ability to have difficult conversations.Work collaboratively with participants, their families, carers and service providers to design, implement and adjust a support plan, including crisis planning and management.Collaborate with the broader services system, in particular mainstream services, and ensure these work in unison with their support plans.ResponsibilitiesThe Service Coordinator is responsible for:WorkplaceContribute to a healthy and safe workplace and understand the responsibilities and accountabilities to yourself and others in accordance with WHS legislation and internal policies, promoting a working environment aligned with those guidelines.Abide by Avenues Australia policies and guidelines to support industry compliance.Ensure all organisational and administrative reporting and accountabilities are completed accurately and in a timely manner.Support Connection and CoordinationSupport clients and carers to understand their funding, goals and objectives.Support clients in developing, implementing, and monitoring a support plan, including crisis management and the risks associated with the breakdown of support.Working closely with clients and carers to identify their needs, personalise support and ensure they receive appropriate services.Promoting and fostering independent decision-making, including choice and control.Activity supporting and managing the sometimes-competing nature of dignity of risk and the duty of care of complex clients.Coordinating and liaising with service providers, reviewing goals and jointly determining new ones with the client.Coordinate the supports in an individuals funded plan by: researching, coordinating and managing complex and intersecting supports to suit individual needs across multiple providers while ensuring client choice and control; and working with mainstream service providers, including negotiating services to ensure service obligations are met.Coaching clients and/or carers to build on knowledge, skills, and decision-making with the aim of greater choice, control and independence in managing their personal choices.Arrange any assessments needed to determine the nature and type of funding required.Decide the budget for each support type and advise any relevant plan manager or service provider of the breakdown of funds.Liaise with providers to establish the appropriate claim categories and attribute funds accordingly.Assist clients to prepare for plan reviews by helping them assess whether they achieved their goals and got value for money. Identify solutions to problems experienced in implementing their plan and to consider new goals.Assist clients to communicate with the funding bodies when required to support their plan requirements.Plan for and pre-empt crisis scenarios by identifying risks and putting strategies in place to mitigate them.Resolve any crisis and breakdown of supports by linking participants with the appropriate services.Actively assess and manage all risks associated with supporting complex clients.OperationalDemonstrate capacity to manage a case load.Demonstrated capacity to work efficiently and to complete administrative tasks.Ensure the completion of appropriate documentation as required.Maintain good quality documentation of clients.Monitor and report client outcomes to the funding body through writing quality reports that are an accurate reflection of the clients support needs.Actively participate in the development, implementation, monitoring, regular review and reporting of client programs with providers including determining risk and preparing assessments, review client records and understand individual needs, review client goals and measure individual progress and document outcomes.Underpinning KnowledgeKnowledge of mental ill-health, disability types and symptoms and/or geriatric health conditions.Willingness to develop sound understanding of choice and control, dignity of risk and duty of care.Willingness to develop working knowledge of the service support providers including mainstream, community-based, and specialist.Willingness to understand the funding arrangements and rules in detail.Willingness to understand the complexities around behaviour support management.Finance and PerformanceEnsuring that care plans are tracked and reviewed 3 months before the end of the Plan.Continually monitor and achieve billable time targets associated with service coordination service delivery.Professional DevelopmentParticipate in monthly supervision with the Line Manager.Undertake annual performance appraisals with the Line Manager and develop and implement your professional development plans.Actively take part in training and skill-building opportunities.Other Duties Conduct other duties as directed.RequirementsCandidates must demonstrate willingness to work towards developing the following competencies:Industry: Broad industry knowledge on the NDIS and/or aged Care sectors, including an understanding of contemporary practices.Technical: Knowledge of best practice person-centred practices.Interpersonal: Excellent person-centred communication skills and ability to build rapport with a diverse range of people.IT Systems: The ability to understand and learn IT systems quickly to support the documentation of essential and sensitive client information.Values: Share our values which are guided and driven by the Avenues Disability Inclusion Framework which underpins our Strategy, policies, service design and culture.QualificationsCertificate IV and above in any qualification related to Community Services, Mental Health, Occupational Therapy, Social Work or any allied health tertiary qualification or other equivalent postgraduate qualification.Current Drivers License and ability to undertake travel within the Perth Metro as required.Low to no prior experience required, as this is an entry level role where significant training and coaching will be provided.Upon acceptance into the role, the candidate will be required to obtain the following:Current Working with Children CheckNDIS Workers Screening and/or National Police ClearanceFlexible hybrid working arrangements policy.Ability to take paid leave in advance.17.5% leave loading.Salary Packaging.Free, independent and confidential access for you and your family to our Employee Assistance Program. #J-18808-Ljbffr
