If you''re looking to make a meaningful impact and provide support to older individuals with a mix of office based and community-based work, we want to hear from you!The Aged Care Assessment Service (ACAS) is designed to deliver client-centred, comprehensive, fair, and timely assessments, empowering older Australians to access Commonwealth-funded aged care services. This role will be largely based in our East Perth office, focused on the triage duties and determining eligibility as a delegate of the Aged Care Act (the Act). In this role, you will also have the opportunity to deliver aged care assessment services to older Australians in the community.Full time / Part time permanent roleCompetitive salary with generous salary packagingThis role will lead the process when triaging a referral for assessment by:Reviewing information from the registration and screening processGathering additional information from the client over the phone on their individual circumstancesDetermining as a delegate under the Act whether the client is eligible for an assessmentConfirming whether the client requires a Home Support or Comprehensive assessment and priority level.If required, issuing an urgent referral to a service provider for time limited supportAssigning the assessment to an appropriately qualified Aged Care Assessor; and scheduling the initial aged care needs assessmentExercise duties as a delegate under the Act with due care, diligence, honesty, and in good faith, being mindful and accountable for decisions made under the ActEnsure performance meets the Department of Health and Aged Care (DOHAC) contractual requirements, including KPI performance that pertains to the Triage Delegate functionSupport the reforms within Aged Care legislation, supporting clients and stakeholders to understand the new aged care needs assessment process and its role in accessing Government funded aged care services.Conduct home support and comprehensive aged care needs assessments for individuals aged 65 and older.Develop and review support plans to assist clients in achieving their goals and living independently.Collaborate with a multidisciplinary team to evaluate clients'' physical, psychological, and social needs and determine their eligibility for government-funded aged care services.Document identified needs and service recommendations for the older adult using tools, templates, and the My Aged Care Portal.ABOUT YOUTo be successful in this role, you will need:Tertiary qualification in a health-related discipline directly related to health, aged care, or a related specialist area. e.g. registered nurses, medical officers, occupational therapists, physiotherapists, and social workers.Current unrestricted registration with the Australian Health Practitioners Regulation Agency (AHPRA) or part of or eligible to be part of a relevant professional association.Demonstrated understanding/experience of the provision of community-based support services to the aged, which reflect a wellness approach with a focus on reablement for clients, including assessment, care planning, monitoring, and evaluation processes.Demonstrated knowledge and understanding of Aged Care legislation and current reforms.Demonstrated knowledge and/or experience in working with a diverse client group, including people who are ageing, people experiencing mental illness, people experiencing homelessness and at risk of, and people with disability and their carers/families.WE OFFERThe opportunity to be part of a diverse and highly motivated team of professionals and support staffConvenient East Perth office locationRewarding work and being part of a Values-based organisation making a difference in peoples livesOngoing support and training, including Professional Development PlansGeneroussalary packaging, up to an additional $18,549 tax-free FBTTrue work/life balance with an additional 3.5 days leave (pro-rata) / ability to purchase additionalleave.Employee Assistance Program for you and your immediate familyTO APPLYRegister your interest today by submitting your curriculum vitaeand a cover letter, addressing how you meet the work-related requirements (in nomore than 2 pages).Short-listing will commence immediately, along with interviewing.
