We deliver holistic, culturally safe primary healthcare, aged care, and community programs across the Kimberley, working through multidisciplinary teams to improve health outcomes and wellbeing across the lifespan. Two Roles, One Purpose We are recruiting for two full-time positions: Support Coordinator and Specialist Support Coordinator. You may apply for one or both roles, depending on your experience. Responsibilities Support participants to implement their NDIS plans Build service connections and reduce everyday barriers Strengthen participant choice, control, and independence Lead coordination for participants with high or complex needs Manage cross-agency responses and risk-based situations Focus on participant safety, stability, and long-term outcomes The Kind of Person Were Looking For You are compassionate, collaborative, and motivated to make a genuine difference.
