Location:Western Australia - Midwest RegionOrganization:Fresenius Medical Care Fresenius Kidney Care WAEmployment Type:Permanent / Full-time (negotiable) About the Role We are seeking an experiencedRegistered Nurseto support our Home Dialysis (Peritoneal Dialysis and Home Haemodialysis) patients within an allocated regional Spoke. This role focuses on patient training, home visits, and ongoing clinical oversight, working closely with our Metro Hub and regional health partners. Renal experience desirable but not required as training will be provided Key Responsibilities Deliver Peritoneal Dialysis (PD) training locally (with potential for HHD training) Provide monthly clinical follow-up for home dialysis patients Conduct home visits (PD and HHD) Coordinate routine blood tests and scheduled PET tests Manage local issues including peritonitis and exit site infections Educate patients, families, and hospital staff on home dialysis care Promote home dialysis as a preferred treatment pathway Maintain accurate electronic clinical records and report regional data Build strong relationships with local health services and stakeholders Deliver culturally appropriate, patient-centred care About You Registered Nurse (post graduate renal certificate/diploma highly desirable) Experience in dialysis (PD & HD experience highly desirable) Knowledge of the dialysis patient journey Ability to work independently and manage competing priorities Strong communication and stakeholder engagement skills Experience or understanding of Aboriginal cultural considerations (desirable) Competent in Microsoft Word, Excel and PowerPoint Why Join Us?
