About the Role Aboriginal and Torres Strait Islander people are strongly encouraged to apply for this opportunity (under section 51 of the Equal Opportunity Act 1984). SACs Deadly Coolangarras (DC) Program will provide needs-based case management to Aboriginal children and young people in Albany aged 11-18 years. The program will work in collaboration SACs Aboriginal Youth Empowerment Program (AYEP), which supports Aboriginal Youths aged 12-25 years of age. Reporting to the Projects & Services Manager, The Lead Aboriginal Youth Worker will lead the Deadly Coolangarras team to support disengaged, disadvantaged and atrisk Aboriginal children and young people to: Receive needs-based case management, tailored to their individual circumstances. Strengthen their cultural identity and connection to Country through knowledge sharing and participation in onCountry activities. Develop practical life skills that will have a positive, sustainable impact on their future. Build their social and emotional wellbeing through a range of activities, including yarning sessions alongside community Elders and SACs other support services. Be warmly referred to external support services as necessary. They will leverage SACs existing relationships with community, carers, local schools and service providers to ensure coordinated support for participants, including practical help like transportation and appointment attendance. The Deadly Coolangarras Program seeks to empower young Aboriginal people to achieve selfdetermination through connection with peers, removal of barriers to services, and support for youth to consolidate connections to culture, while promoting education and positive social participation. About You You are a motivated and committed person that essentially has: Formal qualifications or equivalent experience in social and community services. Demonstrable leadership experience and supervision of employees within a team. Experience in similar intensive case management, assisting young people or with crisis response social support services. Demonstrated ability to lead, liaise, consult and form cooperative and purposeful relationships with staff members, community, government agencies, service providers, ACCOs and other stakeholders. Highly developed interpersonal, communication and negotiation skills and the capacity to build and maintain relationships with a diverse range of people. Demonstrated experience in the development of case plans structured to achieve identified goals. Demonstrated use of organisational processes and systems to ensure that all documentation requirements are completed in a professional and timely manner. And desirably has: Demonstrated awareness and knowledge of working with the target cohorts and broad knowledge of issues affecting young Aboriginal People and the wider Aboriginal community. Mandatory Prior to commencement you must have: A current WA Drivers license. A Prior Offender History Check no older than three months. A current Working with Children Check. Key Responsibilities Promote SACs Deadly Coolangarras Program to service providers, stakeholders and community across the Central Great Southern; Implement and deliver intake referral processes with community; Provide initial and ongoing assessment of client needs, including risk assessment and case planning; Provide proactive, persistent, reliable, practical and comprehensive support in a manner that is respectful of the dignity and privacy of individuals; Contribute to identification of issues and setting of goals and tasks that are mutually agreed with and achievable for clients; Facilitate multiagency case management meetings and cooperate with other services as necessary and appropriate to support positive outcomes for clients; Provide warm referrals of clients to external service providers to access specialist services; Provide warm referrals of clients to internal SAC programs; Employ a culturally safe, trauma informed approach; Ensure that practices are compliant with standards of care and sector standards; Provide respectful and flexible services that are appropriate to the range of cohorts accessing SACs Deadly Coolangarras Program; Adopt policies and practices that enhance access and are culturally safe and appropriate for the target cohort of clients; Accurately maintain, review and analyse client data in accordance with relevant requirements; Provide other reports as required and as negotiated with funding bodies and referral agencies; and Ensure information and support is provided to program clients in ways that they can understand. Develop, coordinate and deliver programrelated activities as required, including but not limited to after school workshops, yarning sessions, onCountry excursions, educational sessions. Support, guide, and collaborate closely with the SAC Aboriginal Youth Empowerment Program Youth Worker and participants Culture Lead & work within SACs established policy framework to support SACs position on the safety maturity curve.
