About the role AsaYouth Case Manager, thisis your opportunity towork within a team of outreach workers across the eastern region of Melbourne. They will work within the Journey to independence model of care and psychologically informed practice framework to providea high levelof support and case management to young people aged 16-25 years who are homeless or at risk of homelessness to access stable,safeand affordable accommodation. Reporting to theClient Services Coordinator,this is apermanent part timerole offering 30.4 hours a weeklocatedinRingwood, VIC. How you will make an impactClient referrals are assessedin accordance withprogram criteria and ongoing assessment and review of client needs,issuesand circumstances frominitialintake to end of support periodClients receive multiple weekly contacts with safety/risk plans developed including mental health and suicide risk assessment, MARAM etc.Individual case, safety and or private rental action plans are developed in collaboration with clients and include a range of interventionsClientsassistedto access support services andestablishcommunity / specialist supportsMaintain strong commitment to working within a trauma and psychologically informed environmentAbility torepresentTSA in strategic working groups, networkmeetingsand complete weekly youth focused outposts across the easter region of MelbourneCase notes are to be completed within 24 hours of each client contact and documents uploaded to TSA information systemsEnsure clients are provided with informationpertaining toThe Salvation Army and our commitment to the safety and inclusion of LGBTIQA+ people WhatyouwillbringRelevant tertiary (Bachelor or higher) qualificationin the area ofSocial Work, SocialWelfareor related fieldDemonstrated experience with accompanying highly developed skills in working with persons experiencing homelessness and who display a range of highly challenging behaviours (essential)Knowledge of service systems, in particular youth, housing and homelessness, Child Protection, AOD, Family Services and Mental Health and Life and Living SkillsExperience in applying Case Management principles and practiceDemonstrated skills engaging homeless persons and willingness toassistin provision of support in areas of domestic violence, drug & alcohol, mental health, living skills and parentingA Current Victorian Driver''s Licenseand an Employee Working with Children''s CheckCompulsory code of conduct, child safe and cultural competency learning modules to be completed and kept up to date What we offerSalary packaging up to $15,900 tax free + $2,650 meal entertainmentbenefit.GenerousPaid Parentalleaveforprimary(12 weeks)& secondarycarers,PurchasedLeaveScheme(up to 8 weeks)and5 days paid leave per year to experience working in other TSA programs or activities.Access to EAP and health & wellness initiativesincluding Fitness PassportOngoing training and development opportunitiesthatenhanceon the jobskills andproficiency.Rewarding and fulfillingpurpose drivencareersthat have positive and sustainable/social impacts.
