About the roleA rewarding opportunity for a passionate Dementia professional to join HammondCare on an Permanent Full-time basis at our Hammondville Site.Dementia Consultant, known within HammondCare Residential as a Dementia Advisor, focuses on improving the lives of individuals living with dementia by applying dementia-specific clinical knowledge and skills in a residential aged care setting. As a key member, you will mentor care staff to enhance their support and implement HammondCare''s Philosophy of care and Case management framework. You will work closely with teams across the organisation, and build capacity, share expert knowledge, and contribute to the development of evidence-based care practices.Some other responsibilities include:On the ground support. Providing dementia specific expert advisory services within HammondCare services, through participation in regular clinical review meetingsCoaching staff, building their ability and capacity to deal with dementia related situations and escalations. Build staff capacity through communication, education, mentoring and coaching in practice; this can include spending time with the team on the ground and lead by example with care support, but also conducting case study discussions and review of documentationsPlan engagement with individuals and teams to achieve agreed practice development objectives and quality improvementsParticipate, contribute and learn as part of the Residential team in team meetings, professional supervision and team activitiesEvaluation and report by utilising HammondCare processes to communicate, monitor and manage work delegations and projects including evaluation and reportingParticipating in projects and behaviour management planningWhat do we need from you?3 years of Proven experience working with people living with Dementia is essentialRegistered Nurse or Allied Health qualification essential.
