Join Each as our Nurse Coordinator for the Victorian Seniors Lifestyle Program and play a key role in improving wellbeing among older Victorians.About the RoleAs the Nurse Coordinator, you''ll be the primary point of contact for participants, working collaboratively with families and multidisciplinary teams to develop and implement tailored action plans. You''ll coordinate person-centred supports across physical activity, nutrition, medication management, social connection, and health coaching, empowering participants to achieve their goals and navigate health and social care systems with confidence.Key ResponsibilitiesEngage participants and conduct goal-setting appointments using a strength-based approachUndertake clinical assessments (Timed Up and Go, Hand Grip Strength, Malnutrition Screening Tool, Patient Activation Measure)Develop, implement, and review individualised goal plansProvide ongoing motivation, health coaching, and regular check-insCoordinate core and supporting services across multiple providersFacilitate communication between participants, carers, GPs, allied health, and community organisationsIdentify and address barriers to care, including access, transport, and health literacySupport referrals, transition planning, and seamless service navigationContribute to clinical governance, quality improvement, and program reportingAbout YouClinical experience in home/community, sub-acute, or aged care settingsAdvanced assessment and clinical care planning skillsStrong interpersonal and communication skillsExperience in person-centred, inclusive service deliveryAbility to work independently and as part of a teamProficient with MS Office and client information systems (e.g., TrakCare, Carelink+) QualificationsTertiary qualification in Nursing (Bachelor or equivalent, unrestricted) Additional Information Why Join Each?Diversity & belonging We value all backgrounds, abilities, and identities and are committed to cultural safetyWork where it suits you Choose from full-time or part-timeOpportunities for professional development and internal career progressionBoost your take-home pay Not-for-Profit Salary Packaging (up to $15,900 tax-free peryear) plus an extra $2,650 tax-free for meals and entertainment.17% annual leave loadingFree, confidential support Our Employee Assistance Program is available for you andyour family.Next StepsTo view the full Position Description click here.
