Comprehensive mental health, community health and dental services are delivered through various programs across the Barwon South West Region, including Torquay and Colac. About the role A member of the Planning and Pathways team should have the skills to be able to collaborate with the Multidisciplinary team to ensure safe, effective and realistic discharge plans, whilst supporting patients (or delegate) decision making authority depending on capacity to do so.Planning and Pathways team are responsible for the facilitation and coordination of patient transfers to other bed-based services and the coordination of referrals to community supports and aged care pathways locally and regionally.
