This position involves coordinating care between acute hospitals and general practitioners (GPs), conducting telehealth consultations, and supporting patients through goal setting and recovery planning.Key responsibilities include:Assessment of patients'' barriers and challenges to recoveryConduct patient reported outcome assessmentsAssist participants to identify achievable recovery goalsLiaise with GPsConduct Scheduled telehealth check-ins to monitor patient progress, recovery milestones and emerging needs.Adhere to trial protocolsAbout youThe successful applicant will bring knowledge and relevant experience and to excel in this role supporting and promoting Cabrini Research.You will have:Registered Nurse, Allied Health Professional; or equivalent clinical backgroundExperience in patient assessment and care coordinationStrong communication skills for liaising with patients and GPsProficiency in telehealth technologiesKnowledge of geriatric care and rehabilitationFamiliarity with functional assessment toolsAbility to create patient-friendly educational materialsTo view the full position description, click hereEnquiries to:Cabrini Administration Officer Research Operations, Gayle Pettigrew Does this sound like your new role?
