You''ll conduct bio-psychosocial assessments, develop individual care plans, and coordinate access to evidence-based interventions such as CBT, DBT, ACT and Motivational Interviewing.This role is pivotal in ensuring seamless, integrated care that addresses both mental and physical health needs, empowering participants and their families with choice, control and connection to ongoing supports.Key responsibilities:Coordinate access to evidence-based psychological interventions and community supportsDeliver interventions including psychoeducation and evidence-based therapies (CBT, DBT, ACT, MI)Identify and manage co-morbidities including physical health and alcohol or drug impactsSupport participants and families to actively engage in their recovery journeyBuild and maintain partnerships with clinical and community stakeholders to ensure integrated service deliveryPromote a positive, collaborative team culture within the consortia and contribute to continuous quality improvementWho we''re looking forRelevant experience in the mental health and welfare community sectorsSkilled in assessment, evidence-based interventions, and care coordination in complex mental health settingsDemonstrated experience supporting people with moderate to complex mental health needsKnowledge of the social determinants of health and mental health recovery principlesExperience working with people from diverse backgroundsCommitted to trauma-informed, recovery-oriented, and culturally safe practiceBrings a proactive, compassionate, and non-judgmental approach to supporting participants and familiesAboriginal, Torres Strait Islander people, Culturally and Linguistically Diverse people and people who identify as LGBTIQA+ are encouraged apply QualificationsQualified and experienced mental health professional (Social Worker, Counsellor, Occupational Therapist, Psychologist or equivalent).Eligible for registration with AHPRA or membership with a relevant professional body Additional Information What''s in it for youAt Each you''ll be supported to be your best.
