The Chronic diseases programs include Cardiac Rehab, Heart Failure, Respiratory, Diabetes, Falls & Balance, Cognitive Disorders, A&TSI Services, Chronic Kidney and Peripheral Vascular Disease clinics. All programs aim to maximise medical therapy and develop the client''s capacity to self-manage their chronic health condition. The clinical nurse provides individual assessment, complex case coordination in conjunction with General Practitioner and Medical Decision Team (MDT), care provision, individual and group education, and group exercise sessions.
