*The Role An opportunity has become available for several Clinical Nurses (Chronic Disease and Post-Acute Programs) to join Gold Coast Health (GCH) where the focus of our strong team is on providing world-class health care to people living in the Gold Coast, and northern New South Wales region of Australia. The Chronic diseases programs include Cardiac Rehab, Heart Failure, Respiratory, Diabetes, Falls & Balance, Cognitive Disorders, A&TSI Services, Chronic Kidney and Peripheral Vascular Disease clinics. All programs aim to maximise medical therapy and develop the client''s capacity to self-manage their chronic health condition. The clinical nurse provides individual assessment, complex case coordination in conjunction with General Practitioner and Medical Decision Team (MDT), care provision, individual and group education, and group exercise sessions.
