Care Team Leader / Care Partner Kalgoorlie (Full Time) Join our team in a vital leadership role supporting the delivery of highquality community care. Working Monday to Friday, 8am4pm, you''ll mentor a team of 810 care staff and lead all care management activities for clients in the Support at Home Program, supporting individuals with low, medium and high care needs. A pool vehicle is available at times, and mileage for personal vehicle use is reimbursed. Key Responsibilities Team LeadershipLead, mentor and support direct care staff to ensure bestpractice service delivery Manage performance, onboarding, communication and timesheet approvals Promote a safe, positive workplace culture and ensure compliance with policies and procedures Allocate resources effectively and respond to staff and client queries Care ManagementCoordinate all care planning and management for Support at Home clients Conduct assessments and develop personcentred care plans aligned with client goals and quality standards Collaborate with clinical and multidisciplinary teams to address changing needs Monitor care notes, service delivery and risks, escalating concerns when required Support clients with My Aged Care reviews and help them understand their package budgets Ensure accurate care management claiming and responsible budget oversight What we are looking for The ideal candidate will have a strong background within the Aged Care Industry. In addition to this, candidates will be able to demonstrate the following experience, skills and attributes:An understanding of health care within a community environment. Demonstrated knowledge and experience undertaking care management activities within Support at Home program Demonstrated experience in leading a team to achieve outcomes. Well-developed communication and interpersonal skills to develop effective relationships, promote services and resolve client/staff issues. Decision making and problem-solving skills to innovatively manage the provision of service delivery. A demonstrated empathetic and flexible approach to manage the sensitivities of client and employee issues. Time management and organisational skills to effectively plan and prioritise work tasks. Knowledge of service provision, allocation and compliance with contractual obligations. Knowledge of manual handling principles with the ability to train others. A Certificate III in Community Support Services is desirable OR minimum two years industry experience and a willingness to obtain Cert 3.
