Opportunity Southern Cross Care QLD has an exciting opportunity for a proactive and selfdirected fulltime Care Partner to join our Community Team working across Miles, Tara, Chinchilla, Meandarra and Jandowae community care area. Based at our Home Care Office in Chinchilla or one of our other local offices, youll manage a caseload of Support at Home and Commonwealth Home Support Programme (CHSP) clients, driving towards exceptional care outcomes. Key Responsibilities Develop personalised care plans according to assessed needs and clients wishes (and their families where appropriate). Develop budgets with clients, utilising funding to meet their needs effectively. Coordinate the delivery of services such as personal care, meal preparation, cleaning, and transportation. Monitor the implementation of care plans and adjust as needed. Monitor clients funding, taking corrective action to ensure services are provided within funding and support the management of any unspent funds. Manage purchasing of clients consumables, ensuring goods are supplied as required. Ensure CHSP services are provided in accordance with CHSP guidelines. Promote clients'' rights, preferences and independent decision making. Model professional behaviours and Southern Cross Cares Known and Loved framework when working with clients and being supportive of support workers. Conduct regular reviews and reassessments as needed to ensure clients'' needs are met effectively. Provide information and support to clients and their families regarding available services and resources. Liaise with healthcare professionals, service providers and community organisations to ensure coordinated care. Maintain accurate records of care management services and document client interactions and service provision. Participate in the outofhours oncall roster. What you will bring to the role Experience in a similar role within aged care or disability services sector. Qualifications in Nursing or Allied Health (desired, not essential). Ability to adapt and respond effectively to changing situations and needs. Practical and solutionfocused approach to problems with a realistic and sensible mindset. Excellent organisation and timemanagement skills with ability to prioritise responsibilities. Welldeveloped attention to detail, ensuring accuracy and thoroughness. Ability to follow processes and create and implement efficient work practices. Reliability: demonstrated ability to consistently meet deadlines and commitments, fostering trust among team members and clients. Excellent communication skills; ability to communicate clearly and effectively with team members and clients. Exceptional interpersonal skills and ability to relate to a variety of people.
