What Youll DoProactively engage with prospective clients and families to promote UnitingSAs aged care services.Lead the onboarding and sign-up of new clients, guiding them confidently through My Aged Care and entry into services.Coordinate services that promote independence and support clients to achieve their individual goals.Co-design and manage personalised care plans in line with My Aged Care assessments, funding, and client aspirations.Collaborate closely with clients, carers, and internal teams to deliver high-quality, goal-oriented care.Monitor service delivery to ensure compliance with funding, quality standards, and legislative requirements.Identify and escalate high-risk situations in line with UnitingSA policies and procedures. What Youll BringStrong relationship-building and communication skills, with confidence engaging new clients and referrers.Experience or demonstrated enthusiasm for client acquisition and service growth within health, community, or aged care settings.A genuine commitment to supporting older people to live well at home.Sound understanding of My Aged Care and community aged care services.Qualification in Aged or Community Support or equivalent qualification.Strong communication, organisational, and time management skills.Familiarity with electronic client management systems.Understanding of Commonwealth Home Support Program, Support at Home Program, and Aged Care Quality Standards.Proficiency in computing skills and ability to learn new systems.A proactive, organised, and outcomes-focused approach.
