Our high-performing team of passionate and talented individuals works collaboratively to deliver great outcomes for our clients.The roleAs part of a team of experienced aged care professionals, your role will involve you visiting aged care facilities to assess residents using the Australian National Aged Care Classification (AN-ACC) assessment tool. This position will be located in or close to Horsham or Stawell and will undertake AN-ACC assessments at residential aged care homes across the Grampians aged care planning region (ACPR) on behalf of the Department of Health, Disability and Ageing . These visits require travel within the Grampians ACPR with the potential for overnight stays for more remote facilities.Comprehensive training and support will be provided (no prior knowledge of the AN-ACC assessment tool is required). Applicants who have been trained as AN-ACC assessors or currently work as AN-ACC assessors will be considered favourably.
