Bundaleer is seeking an experienced Clinical Case Manager to join our Home Care team and provide high-quality clinical oversight and governance for clients with clinical care needs. In this full-time role, you will report to the Home Care Manager and play a key part in ensuring safe, evidence-based clinical practice across our home care services.The RoleAs the Clinical Case Manager, you will be responsible for overseeing clinical assessments, complex care planning, and clinical governance within the home care environment. You will provide expert guidance to non-clinical Case Managers and home care workers, ensuring clinical risks are identified, managed, and mitigated. You will:Conduct comprehensive clinical assessments and develop client-centred care plansLead the clinical governance aspects of home care, including incident management, risk assessment, and mandatory reportingProvide clinical mentoring, education, and competency assessments for home care staffOversee delivery of complex clinical care in accordance with scope of practice and evidence-based guidelinesEnsure all care aligns with Aged Care Quality Standards, including clinical governance (Standard 8)Support investigations of clinical incidents, complaints, and hazardsBuild strong partnerships across multidisciplinary teams to support safe, high-quality clinical careAbout YouYou are an experienced clinician and registered nurse who enjoys working autonomously while supporting a broader care team.
