You will provide acute and post-acute nursing care to eligible patients, including those in Hospital in the Home (HITH) and the Aged Care Outreach Service (ACOS). This role also supports the implementation of innovative care models and plays a key part in strategies aimed at reducing avoidable hospital admissions and presentations.
