It is 500km north of Alice Springs and hosts a major regional hospital, a university, schools, shops and more.About The RoleAnyinginyi Health Aboriginal Corporation has an exciting opportunity for a full time Registered Nurse Diabetes Educator to join their Parlpuru Ninji Kari Health Services Section, in Tennant Creek. This role is offered on a Full Time basis. Reporting to the Clinical Services Manager, through to the Health Services Section Executive Manager, the Registered Nurse Diabetes Educator requires strict confidentiality, initiative and innovation.This position necessitates strong relationships with clients, effective collaboration with a multi-disciplinary team of colleagues within the Health Services Section and with other service providers, and an holistic philosophy to overall good health and wellbeing. The responsibilities aim to improve Aboriginal health in Tennant Creek and the surrounding region.More specifically, some of your duties will include (but are not limited to):Contribute to improving health outcomes for Aboriginal and Torres Strait Islander people with chronic health conditions through better access to coordinated and multidisciplinary care.Contribute to closing the gap in life expectancy by improved access to culturally appropriate mainstream primary health care services for Aboriginal and Torres Strait Islander people.Assist Aboriginal and Torres Strait Islander people to understand diabetes and how to manage it.Advise on the importance of following a care plan, including structured support for diabetes self-management and care plan compliance.Provide support in identifying signs that the client''s condition may require further assistance from a health professional.Maximise community and individual self-reliance, self-care and participation.Involve collaboration with other agencies to support an integrated referral and advocacy system.Involves documentation and procedural adherence, including Medicare billing.Administration and generalIdentify and maintain a register of all people within Anyinginyi''s service delivery area with diabetes.Maintain high quality of practice knowledge, skills and expertise through active participation in professional development including current registration with AHPRA, meeting continuing professional development standards, and meeting any other Mandatory Registration Standards.Provide advice to clients on diet, smoking, alcohol, other drugs and other lifestyle issues which relate to health and wellbeing.Provide capacity building education and information sessions to community.Involve traditional elders and leaders in promoting health and wellbeing within the community.Work within limits of confidentiality and privacy appropriate to this portfolio.Participate in staff training and development, site-based team meetings and other relevant activities.Travel to remote communities.ClinicalMaintain the Anyinginyi client registration in accordance with client information systems Communicare as well as Medicare Closing the Gap (CTG) e-PIP guidelines.Work closely with GPs and other health professionals to ensure services meet contemporary standards for high quality care and diabetes management, including telehealth.Provide high quality diabetes nursing care to individuals with diabetes, their families and support network.Provide high quality diabetes education to individuals with diabetes, their families and support network.Provide high quality healthy living education with diabetes, their families and support network.Be familiar with and competent in the use of standard basic medical equipment.Dispense pharmaceutical therapies, including vaccines and prescribed medications for common conditions.Assist in implementation of quality assurance and other evaluation programs to ensure a high quality of clinical care.All clinicians are expected to work as part of the team in a variety of clinical activities, including related duties as needed.Community CareWork closely with Aboriginal Health Practitioners to improve engagement and access for clients to health services and necessary recalls.Participate in addressing social and emotional wellbeing issues of the community in relation to diabetes activities.Deliver health education and health promotion.Facilitate and participate in opportunistic and community screening activities.Visit remote communities as required and as negotiated.Management and TeamsAdvocate for rights and needs of individuals, families and communities on health-related issues.Demonstrate safe work practices.Provide informal training and education for AHPs, Doctors, RNs and other staff where relevant.ResearchAccurately record and collect data on community health.Contribute to clinical reports and research activities.About YouTo be considered for this position, you must be a Registered Nurse with current AHPRA Registration, with a Graduate Certificate in Diabetes Education and/or relevant experience providing clinical care and education to individuals with diabetes. Experience working with an Aboriginal organisation or communities will be highly regarded but is not essential, provided you can demonstrate understanding of Aboriginal Primary Health Care issues and the social determinants of health in our communities. A thorough understanding of the primary health care system and issues relevant to closing the gap in health outcomes for Aboriginal and Torres Strait Islander peoples is required. You should be a confident communicator with the ability to conduct 1-on-1 and group sessions to educate clients and staff on best practice for diabetes health management, and be innovative in responding to changing health needs while incorporating local values and protocols.Please note: A National Police Clearance and Ochre Card (NT Working with Children card) are required prior to commencement.
