Our office is based in Warabrook but we have clients all throughout Newcastle, Lake Macquarie, Maitland, Cessnock, and parts of the Upper Hunter. Case Management experience is preferred and Diploma of Community Services or Cert IV is essentialYour main duties will include:Assessing client needs, developing a collaborative, strengths-based Care Plan with the client and their carer/representative and implementing services which will meet their individual needsEstablishing the Care Plan within an allocated budget, monitoring the budget and ensuring services adhere to the total budgetFacilitating referrals and work collaboratively with Nursing, Allied Health services (internal and external) and Medical Practitioners to address identified clinical needs.About youRelevant tertiary qualification in the community sectorDemonstrated experience in the provision of case management servicesDriver''s LicenceHighly developed communication skillsConfidence using computer-based systems #J-18808-Ljbffr
