You''ll be part of a passionate team of over 120 staff committed to culturally inclusive, person-centred care.What We''re Looking ForProven experience in Home Care services (HCP)Strong interpersonal and coordination skillsSolid understanding of Aged Care and Community Care Quality StandardsStrong verbal and written communication skills, with the ability to influence and build rapportA proactive mindset with problem-solving skills and keen attention to detailCultural awareness and sensitivity in a diverse workplace environmentA customer-focused approach that puts client wellbeing at the centre of everything you doKey ResponsibilitiesReporting to the Care Partner Team Leader you willBuild strong relationships with clients and their families through regular check-ins to ensure satisfaction and address any concernsConduct comprehensive in-home assessments and develop tailored care plans that support independence and wellbeingCollaborate with internal teams and external providers to ensure seamless, culturally responsive service deliveryMonitor client needs and adjust services proactively to reflect changes in health or circumstancesMaintain accurate and up-to-date client records in line with compliance and quality standardsGenerate regular reports to support management oversight and continuous improvementManage Home Care Package (HCP) budgets to align with client goals and care prioritiesSupport clients transitioning into care packages and assist with smooth transitions into residential care when neededApplications must be accompanied by a cover letter addressing the applicant''s suitability for the role.Salary Packaging available.Fleet car and mobile phone includedGreat team with a diverse and inclusive culture.Employee Assistance Program.We value a diverse workforce and strive to ensure our workplaces are welcoming and enabling to people of all backgrounds.Only Shortlisted applicants may be contacted.
