Job Description

Role OverviewThe role of Care Partner works within the Mission and Mission in Action of HammondCare and plays a pivotal role in supporting older adults to live independently and well in their own homes. The role is responsible for clientcentred care planning and clinical care management and supporting the Client Care Operations Manager in achieving strategic objectives and growing the clients we care for at HammondCare.This role requires a deep understanding of aged care services, a clientcentred approach, and strong communication and coordination skills. You will work closely with clients, families, and multidisciplinary teams to develop and implement personalised care plans that align with each client''s unique needs, preferences, and goals.Key Focus AreasAssess and understand individual client needs, including clinical needs and preferences to develop tailored care plans that align with service expectationsCoordinate and oversee client centred care planning to ensure services are appropriate, responsive and personfocusedAssist clients in developing and managing a budget that aligns with their funding, care needs, and service prioritiesManage and monitor the delivery of inhome support services ensuring compliance to Aged Care Quality StandardsBuild and maintain strong client relationships by providing ongoing support, communication and guidance throughout their care journeyCollaborate with multidisciplinary teams to document, review and ensure compliance with care plans and all related activitiesEngage in ongoing care discussions with clients, carers, and other key stakeholders, facilitating case conferences as neededProvide client education using a reablement and health promotion approach, including:Supporting informed decisionmakingProviding independent advice, information, and resourcesAssisting with system navigation and service linkageIdentify and problemsolve issues and risksAdvocate for clients, ensuring their views, rights, and concerns are acknowledged and escalated when necessaryAssist clients in providing feedback and complaints and take an active role in managing and resolving concernsCommunicate clearly and intentionally with staff, clients, and families to ensure transparency and engagementManage client care in alignment with HammondCare''s Mission, Policies and Procedures, Strengthened Quality Standards, and the Support at Home Program ManualParticipate in continuous quality improvement initiatives to enhance care services and client outcomesQualificationsCommitment to engage and align with HammondCare''s Christian Mission, Motivation and Mission in Action.Universitylevel qualification in a relevant healthrelated discipline, for example, Bachelor of Social Work, Physiotherapy or Nursing OR Certificate IV in Aged Care, Certificate IV in Disability, Certificate IV in Community Services, Diploma of Community Services (Case Management) or Diploma of Ageing Studies and ServicesExperience in aged care, home care, or community care, with a passion for clientfocused service deliveryStrong ability to develop, implement, and review personalised care plansExcellent communication and relationshipbuilding skills, with a commitment to advocacy and personcentred careKnowledge of Aged Care Quality Standards and compliance requirementsExperience working collaboratively with healthcare professionals, carers, and external providersStrong problemsolving, organisational, and coordination skillsExperienced in operating effectively in a multisite environmentA current driver''s licence and safe driving record.
