This role is based in Wudinna and Elliston and plays a vital part in supporting older people to remain living independently at home by delivering comprehensive care coordination under Commonwealth funded home support programs, including Support at Home and the Commonwealth Home Support Program.You will work closely with clients, carers, service providers and multidisciplinary teams to assess needs, coordinate services and review outcomes, ensuring care is person-centred, culturally appropriate and aligned with the Aged Care Act 2024.
