As we continue our rapid growth, we are seeking an experienced and people-focused Head of Care Coordination to lead our Care Coordination function and ensure our operations scale with quality, efficiency, and heart. The Opportunity The Head of Care Coordination is responsible for the leadership, performance, and continuous improvement of the Care Coordination team. This role owns the operational backbone of client scheduling and service delivery, ensuring the right clinician is in the right place, at the right time, every time. Reporting to the executive leadership team, this role works closely with clinical leaders, care partners, software engineering and internal stakeholders to optimise processes, build capability within the team, and deliver an exceptional experience for both clients and clinicians. This is a full-time role based at our head office in South Yarra. Responsibilities Leadership & Team Management Lead, coach, and develop the Care Coordination team Set clear expectations around performance, quality, productivity, and client experience Build a strong team culture aligned with Nimbus Health''s values Oversee onboarding, training, and ongoing development of team members Provide escalation support for complex client, clinician, or scheduling issues Operations & Scheduling Oversee the scheduling of all client appointments across regions and disciplines Handle incoming calls and email correspondence, acting as the key liaison between care providers, clients and clinical staff Ensure optimal utilisation of clinical resources while maintaining flexibility and quality of care Anticipate demand and proactively adjust resourcing and scheduling strategies Work closely with clinical leaders to ensure continuity of care and transparency of client information Ensure operational coverage and service levels are consistently met Address any scheduling issues or conflicts promptly and effectively Process Improvement & Quality Design, implement, and continuously improve care coordination processes and workflows Develop and maintain standard operating procedures and best-practice guidelines Monitor operational metrics including utilisation, responsiveness, quality assurance, and client satisfaction Identify inefficiencies and lead data-informed improvements across systems, tools, and processes Ensure compliance with regulatory requirements, internal policies, and industry standards Systems & Reporting Oversee the effective use of scheduling, CRM, and ticket-based systems Ensure data integrity and accurate documentation across all care coordination activities Produce regular operational reports and insights to support decision-making and growth planning Stakeholder Collaboration Act as a key liaison between Care Coordination, clinical teams, care partners, and leadership Support operational alignment during periods of growth, onboarding new partners or service lines Contribute to cross-functional projects and initiatives as Nimbus Health continues to scale Work alongside our Engineering team to implement process improvement initiatives About You Proven experience leading operational or coordination teams in healthcare, community services, or complex service environments Proficiency in using relevant software applications and systems for scheduling, data management, and reporting Strong people leader with a coaching mindset and the ability to bring structure to fast-paced environments Highly organised and systems-focused, with a passion for process improvement Confident managing scheduling, capacity planning, and competing priorities Comfortable working with data, systems, and performance metrics Clear, empathetic communicator who builds trust with both internal teams and external partners Driven by purpose and motivated to make a meaningful difference in people''s lives Desirable Bachelor''s degree or Diploma in Commerce, Business Administration or a related field What We Offer Competitive remuneration Health and Wellness Program including EAP, gym and health insurance discounts Monthly Team Connect Lunch Allowance Strong focus on work-life balance Head office environment designed for collaboration and connection, including weekly meditation and yoga classes Open communication, autonomy, and the opportunity to shape a growing organisation Regular social events and a genuinely supportive team culture If this sounds like you, we would love to hear from you!
