About the Transition Care Program The Transitional Care Program provides goal-oriented, time-limited, and focused care to support older people following a hospital stay. The program aims to help clients regain independence and return home, rather than prematurely entering residential aged care. Services include: - Nursing support - All services are designed to maintain and improve physical and cognitive functioning.
