About the Role The Support Coordinator plays a vital role in assisting participants to understand, implement, and maximise the benefits of their NDIS plans. The role focuses on building capacity, connecting participants to supports and services, and ensuring each participants goals and aspirations are achieved. The Support Coordinator is expected to promote client choice, independence, dignity, and fulfilment while maintaining strong professional relationships with participants, their families, and other stakeholders. If you''d like to be a force for change, apply now! Responsibilities Assist participants to understand their NDIS plans and funding categories. Develop, document, and implement support coordination plans tailored to participants needs and goals. Connect participants with suitable service providers, mainstream supports, and community opportunities. Capacity Building & Advocacy Build participants capacity to selfmanage and make informed decisions. Provide guidance on navigating NDIS systems, reviews, and reporting requirements. Advocate for participants rights and preferences, ensuring their voices are central to decisionmaking. Collaboration & Stakeholder Management Develop and maintain strong working relationships with participants, families, providers, and relevant community organisations. Coordinate with health professionals, allied health, and other supports to ensure holistic care. Support conflict resolution between participants and providers if issues arise. Compliance & Reporting Ensure compliance with NDIS Practice Standards, Montessori Care policies, and other regulatory requirements. Monitor service delivery and provide feedback to ensure quality outcomes. Maintain accurate records, progress reports, risk assessments, and outcome tracking for each participant. Identify innovative ways to improve service delivery and participant experience. Seek out best practice support strategies and share learnings with the wider team. About You Experience in disability, aged care, or community services. Strong interpersonal skills with a heart for advocacy and support. Proven leadership ability in managing small teams. Techsavvy: confident using CRMs, case notes, and digital systems. Full Australian Drivers License + own car. Valid First Aid and NDIS compliance (or willingness to obtain). A problemsolving mindset.
