DN and HITH, whilst independently coordinated and with dedicated workforces, share the administration assistant resource.DN provides district nursing care to clients under the Commonwealth Home Support Program (CHSP), Victorian Home and Community Care-Program for Younger People (HACC-PYP), Transition Care Program (TCP), Cancer Care @Home and is a service provider to Department of Veteran Affairs (DVA) eligible clients.HITH is an acute NWAU-funded, medically-led and progressive, multi-disciplinary team providing hospital-level care at home via a bed-substitution and Emergency Department-diversion model. Co-located on HITHs virtual ward is a 2-bed Palliative Care @Home service which partners with HITH nurses to plan and deliver palliative care to patients at home.
