Bottisham Medical Practice is seeking an enthusiastic andmotivated Practice Nurse with a specialism in Diabetes to join our team. This is a permanent role working 17.5hrs perweek on a Wednesday and Friday which will report to our Advanced NursePractitioner Main duties of the job Working in accordance with the professional code of conduct as regulated by the Nursing and Midwifery Council (NMC), the post holder will: Provide nursing treatments to patients both independently and in participation with nurse colleagues and general practitioners. Provide assessment, treatment, screening, health education service and advice to patients of the practice. Manage care programmes directly to patients. Deliver a holistic approach to patient services and initiate and manage a methodical assessment of patient and service needs, including public health. Assist the Practice Manager and Senior Practice Nurse in the development of practice clinical protocols for the clinical team. Participate in an effective, quality driven Practice Nurse Service, liaising and working effectively with other agencies. Develop a working environment that promotes health, safe and effective work outcomes. To have a positive attitude and approach towards helping solve the problems and challenges that face General Practice today and be part of the process to help the Practice achieve its goals. The post-holder will have the required level of training and competence and will work within patient group directions where these are available. A nurse with prescribing qualifications will use these appropriately. Undertakingdressing of complex, including post-surgical wounds and leg ulcers Ensure trainingand skills are maintained in the use of Doppler machines Wound Care andmanagement Assessing routinewounds including trauma Assessing tosupport good wound management in complex situations (e.g. use of Dopplertechnique) Identifyingsignificant abnormalities Obtainingappropriate information using a lifestyle questioning approach andidentifying appropriate health promotion issues Working withpatients to develop a management plan where health problems or potentialhealth problems are identified Recognising issuesand gathering sufficient information to refer (e.g. drugs;domestic violence; child protection; vulnerable adults; seniorpatients; social problems) Health screening Recognising issuesand gathering sufficient information to refer e.g. women''s and men''shealth, sexual health, older people Undertakingmonitoring tasks e.g. smearsand providing advice as appropriate Providing in depthmonitoring and advice as appropriate to the level of the post-holdersspecialty and working with the patient in deciding on management plans. Travel health Carrying out RiskAssessment of vaccinations needed for travel destinations Administeringinjections and providing guidance in accordance with PGD guidelines Identifyingproblems Immunisation - adult andchild Working withpatients with more complex immunisation issues. Administer injections under an individualised prescription or PatientGroup Direction. Ensure safe storage, rotation and disposal of vaccines and drugs First contact Workingindependently or alongside the general practitioner and making decisions and - or recommendations as appropriate. Providing routineear care Dealing with morecomplex problems and referring to a Doctor if prescribing is appropriate. Mental health Communicating withkey workers Administeringappropriate prescribed therapies and monitoring for side effects. Emergency Management Ensurecompetency in anaphylaxis and resuscitation techniques by maintaining andupdating CPD. Infection control Supportpatients and colleagues in adopting sound infection control measures Applyinfection control measures within the practice according to local andnational guidelines - This includes Universalhygiene precautions Thecollection and handling of laboratory specimens Segregationand disposal of waste materials Reportingand treatment of sharps injuries Dealingwith blood and body fluid spillages Generalresponsibilities will include: Administration of the Clinical Quality andOutcomes Framework - QOF Assistin accurate recording of data input to support the QOF.
