Home Support Assessor page is loaded## Home Support Assessorlocations: Morayfield, Queenslandtime type: Full timeposted on: Posted Todayjob requisition id: JR100103**Join an Organisation dedicated to Closing the Gap in Indigenous Health**The Institute for Urban Indigenous Health Ltd was established by its founding members to provide a coordinated/integrated approach to the planning, development and delivery of comprehensive primary health care services for Aboriginal and Torres Strait Islander populations within the Southeast Qld Region.The focus of IUIH Aged Care team is to work with the local Indigenous community and relevant key stakeholders to increase access to comprehensive care and provide well-coordinated, quality in home care services to eligible aged care clients.This position plays a key role within an innovative pilot program delivering culturally safe, person-centred aged care Assessment services Assessment Services to older Aboriginal and Torres Strait Islander people, operating in one or both of the Caboolture and Sunshine Coast Aged Care Planning Regions (ACPRs). The role is responsible for conducting culturally appropriate Home Support Assessments for eligible older Aboriginal and Torres Strait Islander individuals, in accordance with the policies and procedures of the Aged Care Single Assessment System.We have a full-time temporary role available.
