About the Role As a Care Manager, youll lead the client journey from their very first enquiry through to ongoing care planning and support. What Youll Be Doing Designing and managing personalised care plans Coordinating with clients, families, caregivers, and health professionals Reviewing goals and ensuring clients voices are always heard Conducting home visits and reassessments Documenting progress and feedback to continually improve care quality Qualifications Experience in aged care or working with seniors Home Care Package and Support at Home experience a big plus!
