About The Opportunity Forster Community Health Service is seeking a Registered Nurse who is passionate about palliative care to join our Generalist Care team. This role offers the privilege of supporting patients and families in their own homes, providing specialist care across the Lower Mid North Coast. About The Role As a Community Palliative Care Registered Nurse, you will: Deliver personcentred care to people living with lifelimiting illness in their homes Provide expert symptom and medication management Coordinate care and manage your caseload with confidence and autonomy Work closely with the Lower Mid North Coast Specialist Palliative Care team to meet physical, emotional and social needs Support patients and families to maintain dignity, comfort and quality of life About You A Registered Nurse with a genuine interest in palliative care Empathetic, resilient and committed to holistic practice Skilled in communication and relationshipbuilding Organised and confident in managing a community caseload A critical thinker who values evidencebased care Comfortable working both independently and within a team Postgraduate palliative care studies are ideal, or you are willing to pursue them. Youll also collaborate with broader specialties such as Wound Management, Cardiac and Respiratory Rehabilitation, Continence, Dietetics, Diabetes Education, Immunisation, Audiometry and Wound Specialist services. The Palliative Care team, including a Clinical Nurse Consultant, nursing staff, a Bereavement Counsellor and volunteers, supports Forster as one of four key regions across the Lower Mid North Coast.
