We are seeking a compassionate, proactive individual who understands that supporting an older person means looking at the whole picture their health, wellbeing, environment, and goals. About the Role In this role, you will take a holistic view of persons individual needs. You will ensure the right supports are in place at the right time, enabling older women to maintain independence, safety and quality of life. Your work will involve building trusted relationships, recognising changes in health or function, coordinating services, and advocating for supports that promote dignity, purpose and meaningful engagement. Your work will include: Ability to think ahead, plan proactively and adapt support as needs evolve Strong understanding of aged care programs, community services and supports that help older people remain independent Capacity to work collaboratively with GPs, allied health and community providers A genuine interest in the life stories of others and a commitment to personcentred, valuesbased care Welldeveloped interpersonal, communication, listening and negotiation skills Leading and supporting a small team This is a part-time position (30 hours per week: no shifts or weekend work). This is a 12month maximum-term contract with the possibility of extension. Some flexibility in hours may be required to meet daytoday needs.
