Just Better Care''s Care Partners play a key role in helping families receive the care and support services they need.Care Partners are responsible for the development, maintenance and review of individualised support plans in consultation with client, their family or guardians and the key health professionals.About us:Across Australia Just Better Care provides in-home aged care and disability support services, supporting people to live independently. For over 15 years, we have been supporting older people, and people living with disability, to live independently in their own homes and stay connected to their local community.Our friendly, compassionate and experienced team makes this possible.Duties and responsibilities include:Day to day management of a client case load, conducting assessments, care planning and provision of consumer directed services to complex care clients within budget guidelines.Ensure appropriate information is provided and explained to the customer at all times regarding the service agreement, including the customer handbook.Where applicable, develop strategies which will address the identified needs of the customer in consultation with their representative/s where appropriate, in accordance with funding guidelines.Provide financial and case management services to customers, and representatives where required.Building professional relationships internally and externally and look for creative care solutions to achieve the best outcomes for clientsEnsure services are delivered are in line with the person centred and consumer directed principles and assist customers to access other alternative services where the services are no longer meeting their needs.Liaise with relevant community based agencies and service providers to ensure an integrated and cohesive approach to service delivery and to ensure standards are met in accordance with program objectives.Monitor customers budgets in accordance with program guidelines.Act on care need recommendations by Health Care Practitioners.Ensure compliance with all Just Better Care policies and procedures, statutory and contractual requirements in relation to management of service delivery.Apply and uphold principles of equity and anti-discrimination in the workplace and adhere to organisational and legislative Health, Safety and Environment requirements.About you:Demonstrated high level verbal and written communication skills, including familiarity with, and confidence in using, the Microsoft SuiteProven relationship management with all internal and external stakeholders, as well as good interpersonal, negotiation and problem solving skillsAbility to work as part of a teamPrevious customer service coordination experience in the health/aged care, non for profit, community services sector is desirableWhat you will need:Criminal Record CheckCert III in individualised support or Diploma of Enrolled nursingPrevious experience in the community care or health/aged care sectorCurrent Australian drivers licence and reliable vehicleNext steps:If you would like to join our growing team and become part of our friendly, professional network please apply today by clicking on Apply.Be careful - Dont provide your bank or credit card details when applying for jobs.
