Were looking for a Service Coordinator to join our team and play a key role in supporting families in the Northern Suburbs (maternity leave cover). Manage client onboarding, including liaison with families and external providers. Complete and maintain all required documentation such as care plans, service agreements and risk assessments. Maintain regular client contact, including home visits and ongoing review of supports. Work with the rostering team to manage staffing changes and ensure appropriate staff allocation. Support staff performance, problem solving, mentoring and participation in recruitment activities. Assist the Director Clinical Operations with operational planning, continuous improvement and professional development requirements. About you Experience supporting clients in the disability and/or health sector Working knowledge of the NDIS. Experience supervising and leading staff. Strong analytical and problem solving skills. Highly competent with technology and able to learn new systems quickly Strong communication and interpersonal skills High attention to detail with a commitment to continuous improvement.
