In this rewarding role, youll deliver high-quality clinical care that supports our clients to remain independent in their own homes, while building meaningful relationships and contributing to a service that truly makes a difference.Part-time - two days - 15.2 hours per week and days negotiableHourly pay rate - $47.62 + Super. + Salary packagingRole Specific RequirementsUndertaking clinical assessments of home care clientsProviding excellent care and service to clients in line with their personalised care plansMonitoring chronic diseases and palliative conditionsWound care, continence care, palliative care, medication management, and management of chronic diseasesAchieving the optimum care for clients by being responsive to their changing needsLiaising with and referring to other organisations as required to provide for the client''s needsAssist the Clinical Lead with other duties, as requiredQualification and ExperienceBachelor of Nursing and registration with the Australian Health Practitioner Regulation Agency (AHPRA)Basic IT and smart phone skills/literacyAbility to exercise initiative and discretionAbility to maintain a high level of confidentialityExcellent communication and interpersonal skills, including writing reports and other associated documentationAbility to work with individuals from a range of social, financial and cultural backgroundsAbility to cope with changes in work demands, to think outside the box and problem solveNDIS Worker Screening CheckAustralian work rights / appropriate visaCurrent National Police Clearance (not older than 6 months)WA Drivers LicenceDementia training or experience in dementia care.Previous experience in community nursingHow to applyApplications should include a resume and preferably a brief cover letter outlining your suitability for the role.
