This is a full-time opportunity for someone who thrives in a fast-paced environment, excels at coordinating care services, and is passionate about driving growth while supporting Home Care Package (HCP) recipients.This role goes beyond traditional case managementit''s leadership in action.About the RoleIn this vital position, you''ll oversee the coordination of services under theSupport at Homeframeworkincluding nursing, allied health, home support, and equipmentempowering clients to make informed decisions and exercise choice and control over their care.Travel across the Northern Rivers region, from Ballina to Tweed Heads, is required.
