Support at Home Care Partner / Case ManagerRight at Home, a global leader in home care, is proud to be one of the largest providers worldwide, with 60 franchises across Australia and over 600 offices globally. Our Melbourne North (South Morang) office is seeking an experienced and dedicated Support at Home Care Partner / Case Manager to join our dynamic and supportive team.About the RoleAs a SAH Care Partner , you will play a vital role in coordinating high-quality, person-centred care for our clients. You will:Build and maintain strong partnership relationships with clients and their families.Provide comprehensive case management tailored to each client.Develop and review individualised care plans that align with client needs and SAH budgets.Re-assess client care plans, goals, OHS assessments, and emergency plans.Maintain accurate and detailed client records and case notes.Recognise, anticipate, and advocate for specific client needs.Collaborate with the Finance team to ensure accurate Services Australia claims.Review and approve monthly client statements prior to distribution.Establish, coordinate, and manage third-party services for clients.Support clients and families to understand and make the most of their SAH budget.Identifying when care needs have changed and assisting in the process of applying for package upgrades to ensure clients receive the appropriate level of support.Collabortaing with a multidisciplinary team to achieve client goals .Ensuring complaince with organisational and program standards.QualificationsMinimum of 2 year experience in care coordiantion and case management or a similar role within aged care or community services .Proficiency in computer systems and CRM platforms.Preferred SkillsAbout You : We are seeking a professional who isExperienced in a similar Case Management role within aged care or community services.A Registered Nurse or allied health professional looking for a rewarding career change.Proficient incomputer and CRM systems.An excellent communicator with strong interpersonal skills.Able to work bothindependentlyand as part of a multidisciplinary team.In possession of a valid driver''s licenceSkilled in communication and able to build strong interpersonal relationships.At Right at Home, we believe in more than just care.
