Our evidence-based, person-centred approach is tailored to each individuals needs, and our multidisciplinary teams work collaboratively to help participants thrive.The RoleAs our Support Coordinator, you will oversee the delivery of high-quality services and lead a motivated team. Your leadership will ensure our services meet the highest standards and reflect Hope Abilitys values and mission.Key Duties and ResponsibilitiesEngage with participants, their families, and carers to explore and connect them with opportunities for personal development, education, and employment.Assist participants in working toward their goals by coordinating supports and services in line with their NDIS plans.Build and maintain positive relationships with community organisations, service providers, and other stakeholders to ensure effective support for participants.Monitor participant progress and review supports to ensure they continue to meet individual needs and outcomes.Keep accurate and up-to-date case notes, records, and reports that clearly reflect participant progress and support activities.Communicate and collaborate with a range of stakeholders including families, community services, government bodies, and other service providers.Take part in team meetings and planning sessions, contributing ideas and solutions to improve services and participant outcomes.Remain adaptable and open to new approaches to support continuous improvement in service delivery.About YouTertiary qualifications in Social Work, Psychology, Nursing (Mental Health), Disability, or a related field.Experience managing or leading teams in disability, mental health, or community services.Understanding of psychosocial and forensic disabilities and complex support needs.Commitment to person-centred and rights-based practices.Excellent interpersonal, communication, and organisational skills.Ability to work independently and as part of a team.Bachelors degree or higher in a relevant field.Experience in NDIS service delivery (desirable).Skilled in community participation and remote service coordination.Experience with complex disabilities and behaviours of concern.Proactive, solution-focused attitude and sound business management knowledge.Why Join Hope Ability?Work in a values-driven, inclusive environment where your efforts matter.Access to ongoing training, development, and clear career pathways.Make a positive impact on peoples lives every day.Competitive salary, company vehicle, and additional benefits.Are you ready to bring positive changes in peoples life?
