Qualifications & Experience Cert III in Individual Support, Mental Health, Aged Care, Community Services, Disability Work or similar. Full Australian Working Rights. Current WA Unrestricted Driver License. National Police Clearance (current within 6 months). NDIS Worker Screening Check (to be obtained if successful and prior to commencing work). Current First Aid, CPR and Manual Handling certificates or willingness to obtain prior to commencing work. Current Working with Children Check (or willingness to apply). Minimum 21 years of age. Willingness to obtain NDIS worker orientation module, supporting effective communication, supporting safe and enjoyable meals, new worker NDIS Induction Module and Covid19 infection control certificate prior to commencing work. Vehicle safety inspection certificate with current vehicle registration & a clean, reliable, roadworthy car with comprehensive insurance. Proof of COVID19 and Influenza vaccinations and/or willingness to provide vaccination certificates as per disability industry standards. Tasks & Responsibilities Support clients to participate in ageappropriate activities and tasks, including employment, inhome support such as personal care and household chores, and community access. Provide direct assistance to each client in accordance with their service plan and as directed. Work in accordance with the medication administration policy, work health and safety procedures, and client rights policies including privacy and confidentiality. Monitor the safety and wellbeing of the clients and report any concerns to the Manager. Ensure completion of appropriate documentation; record relevant information on support provided in client notes and communication books as required. Work with clients, family/carers, relevant case managers and other professionals to support the client''s individual goals. Liaise extensively with other staff to ensure the overall smooth and consistent running of the service. Notify the service if you are unable to work your rostered shifts, with as much notice as possible. Report any quality and safety incidents or hazards at work to line Manager or Supervisor.
