Even if you only meet some of the criteria, wed love to hear from you. About This Program Consumer Directed Services (CDS) refers to services which are led by and funded by individual consumers. This includes services such as the National Disability Insurance Scheme (NDIS) services, Life Time Care, My Aged Care services & Community Care. These services provide support, care and access to services and technology for Australians with disabilities as well as their families and carers. About The Opportunity As a Support Coordinator, you will work with participants to implement their NDIS plans and coordinate services that support their recovery and wellbeing. You will collaborate with participants, families, carers, service providers and other stakeholders to help individuals achieve their goals. Recovery Coaches support participants with psychosocial disabilities in their recovery while coordinating supports in line with their NDIS plan and individual goals. The role focuses on helping participants build connections, develop relationships and strengthen support networks so they can gradually take greater responsibility for their recovery. You will need: Relevant qualifications or commensurate experience in working in a support role for people with mental health issues and Experience in the human services or related sector including individual and family support worker and intake and assessment experience Current valid Drivers License and the ability to undertake travel for the role Current Working with Children Check and NDIS Workers Screening Check Right to Work within Australia Satisfactory National Police Records Check (less than 12 months old) NDIS Worker Orientation Module Certificate About Us Wellways connects people, strengthens families, and transforms communities for individuals of all ages experiencing disabilities or challenges to their social and emotional wellbeing.
